Expressions of Life offers many different services





A Residential Service that is geared to enhance quality living for people with disabilities in the most natural home setting. This service is not limited to natural families but includes surrogate families as well. Home Base is a service that enhances quality of living, inclusion, and meaningful relationships. We as Service Providers promote this type of living arrangement because it enables the person with the disability to become or remain a valued part of the family. When asking a person with a disability why he or she has chosen to live in a home base arrangement, their response is that they now have an important role in the family. In this setting the family structure is maintained, twenty four seven, there is assistance with medication, health care coordination, Assistance with accessing community resources and proactive planning for all aspects in life including and not limited to Family vacations. This residential support service allows for the individual to take ownership of their lives.



This service is provided on a short term basis because of the absence or need for relief of those persons normally providing the care in their home.

We provide on time and immediate coverage when respite or emergency care is needed. Respite activities may include outings of the persons choice and in home substitute care.



Services provided to assist the individual with activities of daily living while providing companionship to acquire, maintain or improve skills. Services range from total personal care to assisting and enhancing an individual's skills and abilities.



Services are designed to increase or maintain the individual’s skills and independence and promote self-advocacy. Independent Living Services are for people who need less than 24-hour staff support per day. Independent Living supports are only provided in the individual’s home or family home in the community. Services include 24-hour on-site response capability to meet an individual’s scheduled or unpredictable needs.



Services assist the individual in accessing other waiver supports and non-waiver activities identified in the Individual Service Plan (ISP). The primary goals of this service are to facilitate greater inclusion in community life and to promote self-determination. Non-Medical Transportation services enable individuals to gain physical access to nonmedical community services andresources promoting individual opportunity and responsibility in carrying out ISP activities.
